Welcome to the UAB School of Optometry Online Application for Admission

Please read all instructions carefully as you complete the UAB School of Optometry online admission application. NOTE: This application requires you to submit a personal essay regarding the development of your interest in becoming an optometrist, about you as a person, your goals and why you believe you are qualified to become an optometrist. It is recommended that you complete this personal essay before filling out this online application. Please limit your personal essay to two(2) standard pages.

Before you begin, please have your credit card ready. You must pay your $75 nonrefundable application fee online by credit card to successfully submit your online application. After your credit card payment is submitted, you will receive a payment confirmation page. Application System Requirements: You will need a web browser that has Javascript activated. Supported web browsers include Internet Explorer 6 and above, Netscape 6 and above, Safari (for Macintosh users), Firefox 2 and above. Do not use AOL's internal browser.
